As a federally qualified health center surveyed and licensed in the Commonwealth of Kentucky as primary care centers, Park DuValle Community Health Center strives to adhere to the highest quality standards of patient care, regardless of insurance or ability to pay.

Our Model of Care: Patient Centered Medical Home
What is a Medical Home?
The Medical Home is how we provide Primary Health Care services. It is:
Patient Centered
We teach and help patients to take care of themselves.
We make sure patients and families are involved in planning care and choosing a health care provider.
We teach patients and families that it is important to tell their health care provider about all of their medical history including visits to specialists and the emergency room.
Coordinated
We make sure that everyone works well together to give the best care including communicating with specialists, hospitals and community services.
Accessible
We make sure that patients get the best care during and after office hours, face to face and over the phone.
Comprehensive
The care team leader is primary care provider (MD, Nurse Practitioner, PA) who cares for the whole patient, including physical and mental health, prevention, sudden and brief or long-term illnesses.
Committed to Quality & Safety
We make sure that patients get evidenced-based care and follow up on results as compared to standardized and expected outcomes.
We are committed to quality improvement using Health Information Technology, computers and other tools that help patients and families to make health decisions based on information they can understand.
What is a Care Team?
The Care Team includes your Primary Care Provider, Nurse, Medical Assistant and Specialists that work together with you and your family to give excellent care focused on the “whole” patient. Together, you and your care team will build a relationship based on trust and understanding. To help do this:
You can communicate with a Medical Home care team member anytime during normal working hours by calling the main office numbers shown on the front of the brochure.
When our office is closed, you may contact your Medical Home team by calling the main office telephone numbers.
1. We will record why you are calling and let the On-Call Provider know, immediately.
2. The On-Call Provider will contact you personally to talk to you.
If you go to any other health care provider, please, let your Medical Home care team know. This will help us to make sure we know about all of your health care needs and give you the best possible care.
